Our practice is no longer in-network for EyeMed or Anthem Vision Plans.  If you have EyeMed or Anthem Vision coverage, we may be able to bill your medical insurance for future visits.  Annual diabetic exams will continue to be billed to medical insurance.  When requested, we will provide patients with itemized receipts in order to request a reimbursement through their EyeMed or Anthem Vision plan.  We will no longer submit claims to these plans for examinations, eyeglasses, or contact lenses.
